**A Note about Self-administered Medications: Students are not permitted to self-carry, or self-dose medications (including cold/allergy medicines, pain relievers, or Asthma Inhalers) unless they have completed a self-administration form. Please see Mrs. Beard for this form to be completed and signed.**

Below are some helpful forms for parents:

Health Assessment Form: All students need to have one on file in the nurse office. Provides information about pertinent health conditions.  Download Here

PPA: All meds whether OTC or RX need to be documented and signed by an MD on this form, along with a parent signature. Download Here

PEEHIP Flu-Shot Consent Form: Students who wish to receive the Flu Vaccine, and are covered by PEEHIP insurance should complete this form. Download Here

Flu-Shot Consent Form (non-PEEHIP): Students who wish to receive the Flu Vaccine, and are not covered by PEEHIP should complete this form. Download Here
